SUMMARY: The Preferred Communities (PC) Case Manager works within the framework of the Preferred Communities grant providing intensive case management services to clients who meet the PC eligibility criteria (mental health, medical, women at risk, etc.). The PC Case Manager ensures clients’ basic and specific needs are met so that they can overcome barriers and make progress towards self-sufficiency. The PC Case Manager ensures compliance with PC grant requirements. This position will also supervise the interns of BSU School of Social Work.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conducts assessments to determine client needs and eligibility for Preferred Communities’ services;
- Maintains frequent contact with assigned clients (at a minimum once a month, and more frequently as needed), including home visits (as needed);
- Assists clients in accessing and linking with appropriate external services and public benefits;
- Coordinates with other ANA staff and community providers to link with services including English classes, childcare, education, health care, immigration legal services, mental health services, addiction services, disability services and benefits, housing services, etc.;
- Assists clients in attending appointments for various services and helps them find ways to get to their appointments independently;
- Provides additional community and extended-cultural orientation (in addition to what was provided during reception and placement period), as needed;
- Ensures that proper language interpretation and translation is provided as required by Office of Refugee Resettlement program guidelines;
- Maintains case files including required documentation for each beneficiary client and keeps accurate and timely case notes as per program guidelines;
- Maintains and updates Preferred Communities database with client information, periodic assessments, resources and gaps, and referrals;
- Closes and transfers cases in a timely fashion;
- Conducts or contributes to outreach efforts to local service providers and access to eligible populations;
- Participates in Preferred Communities program network meetings;
- Provides regular reporting to ANA management and funders about the Preferred Communities program;
- Regular and predictable attendance is an essential function of this position
- Manage ANA’s Intern/s of BSU School of Social Work; recruit, screen, interview and supervise;
- Conduct an orientation for BSU School of Social Work interns;
- Continue collaboration between ANA and BSU School of Social Work
- Be a point contact in case of questions and concerns;
QUALIFICATION REQUIREMENTS:
- Ability to work effectively with refugee families of diverse backgrounds;

- Skills and experience in developing services that are culturally and linguistically appropriate;
- Organized and attention to detail;
- Ability to take initiative and work independently when required while maintaining a team orientation toward the provision of services;
- Ability to interpret and follow applicable rules and guidelines;
- Excellent verbal and written communication skills;
- Completion of a criminal history background check post offer of employment;
- Access to a reliable vehicle and ability to provide transportation for self and refugees;
- Provide verification of current state-required liability insurance;
EDUCATION:
- Master’s degree in social work preferable MLSW.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Genuine interest and support of Jannus’ commitment to diversity, race, equity and inclusion integrating these concepts into daily activities to create healthy workplace environments
- Conduct Employment Assessment Plan with each employable adult to assess skills, experience, etc.
- Strategize with each family on achieving economic independence;
- Monitor Case and FSSP plan and update goals towards self-sufficiency and employment;
- Develop a network and actively seek new employment opportunities for refugees;
- Connect eligible clients with training and education opportunities that will better prepare them for the workplace;
- Obtain all work and termination verifications from employers;
- Conduct required follow-up calls with employers (2wks and 90 days) and clients after job start to ensure retention;
- Provide support services to employers if necessary to enable the refugee to retain job;
- Assure transportation arrangements for refugee clients after job start;
- Complete 90-day retention reports;
- Maintain complete and accurate case files for all refugees enrolled in employment services;
- Conduct intakes with clients wanting to enter the extended employment/subsidized employment services to assess needs;
- Work with clients enrolled in extended or subsidized employment services;
- Complete reports and budgets with clients as needed;
- Conduct periodic review of data to ensure it is current and complete;
- Other duties as assigned.
QUALIFICATION REQUIREMENTS:
- Proficient in computer usage;
- Ability of enter data and perform calculations accurately;
- Good communication skills and ability to relate well to the public;
- Ability to take initiative and work independently when required while maintaining a team orientation;
- Excellent written and verbal skills;
- Multicultural work or volunteer experience, preferably with refugee populations;
- Ability to teach and motivate individuals toward actions;
- Creative thinker and problem solver;
- Ability to provide transportation for self and refugees;
- Ability to speak other languages such as: Swahili, Arabic, Dari, etc.

- A Bachelor’s degree in social work, business, etc. is preferred.
